How to remove facial wrinkles with botox fillers injections, let’s talk about wrinkles first.


Dynamic wrinkles are caused by contraction of muscles that are attached to the overlying skin. Each muscle contraction causes the skin to bunch together, forming lines between the bulk of the muscle. The most common locations for dynamic wrinkles are frown lines between the eyebrows, horizontal lines across the forehead, and lines extending from the outer corners of the eyes, often referred to as “crow’s feet”. Weakening these muscles will cause a reduction in the severity of the facial wrinkles, or can prevent them altogether when muscle weakening is done before wrinkles form. In addition to surgery, which can permanently weaken some of these muscles, applying botox fillers injections into the specifically targeted muscles will temporarily decrease the action of the muscles for 3-4 months thereby eliminating wrinkles or causing a softer appearance to already formed dynamic wrinkles. Most people have heard of breast implants. But what is a butt implant, and how do they differ? Buttocks implants are made of a solid, but soft silicone. The implants are very strong and will not rupture or leak into the body. The surgeon will make an incision about 2”-3” long in the crease of the buttock. The scar will be invisible placed in the crease between the buttocks. The implant will then be inserted either submuscular (under the muscle) or sub-fascial (above the muscle). The Submuscular Buttock Implant is the best choice if you are looking for more fullness in the top half of your buttocks. To insert the buttock implant, the surgeon has to make an incision either in the gluteal crease or in the natal cleft. Although the first region carries a much lower risk of infection, cuts are more commonly placed in the groove between the butt cheeks where scars are not visible. ¿Cómo hace el cirujano para reconstruir el seno con implantes?

Los implantes se pueden poner por debajo de la piel y de los músculos del pecho que quedan después de la mastectomía. Esto se hace generalmente como parte de un procedimiento de dos etapas.

En la primera etapa, el cirujano coloca un dispositivo llamado expansor debajo del músculo del pecho. El expansor se va llenando lentamente con solución salina durante las consultas al médico que tienen lugar después de la operación. En la segunda etapa, después de que el tejido del pecho se ha expandido y ha sanado lo suficiente, se quita el expansor y se lo reemplaza con un implante.  En general, el tejido del pecho está listo para el implante entre 6 semanas y 6 meses después de la mastectomía.

Los expansores se pueden colocar ya sea como parte de la reconstrucción inmediata o en una cirugía posterior. Existe una tercera etapa opcional en la reconstrucción del seno, en la cual se crea un pezón en el seno reconstruido.


¿Qué es la reconstrucción del seno?

Muchas mujeres que se han hecho una mastectomía—una operación quirúrgica para extirpar todo el seno con el fin de tratar o prevenir el cáncer de seno—tienen la opción de hacerse más cirugías para reconstruir la forma del seno que fue extirpado.

La cirugía de reconstrucción del seno puede hacerse de forma inmediata o se puede postergar. Cuando la reconstrucción se hace de inmediato, el cirujano inicia la primera parte del proceso durante la misma operación de mastectomía. Se puede usar un método llamado mastectomía conservadora de piel para preservar una cantidad suficiente de la piel del seno que alcance para cubrir la reconstrucción.

Cuando se pospone la reconstrucción para más adelante, el cirujano inicia la primera etapa del proceso cuando el pecho ha sanado tras la mastectomía y después de que la mujer haya terminado la terapia adyuvante (quimioterapia, radiación, etc.)

Una tercera opción es la reconstrucción inmediata postergada. Este método consiste en colocar un expansor de tejido debajo de la piel durante la mastectomía para preservar el espacio para el implante, mientras que se analiza el tejido que se ha extirpado. Si el equipo de cirujanos decide que la mujer no necesita radioterapia, se puede colocar sin demora un implante en el lugar donde estaba el expansor de tejido. No obstante, si la mujer debe recibir radioterapia después de la mastectomía, la reconstrucción del seno puede postergarse hasta luego de finalizar la radioterapia.

Los senos se pueden reconstruir con implantes (de solución salina o silicona) o de tejido autógeno (esto quiere decir, de tejido de otra parte del cuerpo de la persona). La mayoría de las reconstrucciones de seno que se realizan actualmente son cirugías reparadoras inmediatas con implantes.

It may be possible to slow down the skin’s aging process by boosting support of the dermal extracellular matrix, a new study finds.

As the skin ages, the extracellular matrix, which acts as a scaffold for skin cells, becomes fragmented. Fibroblasts shrink and less collagen is produced causing the support structure to decline.

The researchers found that injecting the space between the skin cells with a filler enhances the structural support of the extracellular matrix.

Investigators injected a dermal filler into the facial skin of 21 volunteers over age 70 during a three month period. Data indicated that the filler stimulated fibroblasts to begin producing more collagen and boosted the support structure of the extracellular matrix. This increased support, the researchers noted, appeared to further stimulate the proliferation of fibroblasts and increase epidermal thickness, according to the abstract. The entire layer of skin grew thicker with more blood vessels, which nourished the cells.

“By altering the matrix using an external filler and increasing the internal pressure, we’ve shown that we can essentially trigger a signal for cells to wake up,” a researcher said. “This shows that skin cells in elderly people have the capacity to respond robustly in a very positive way to alterations in the mechanical property of their environment. We still need to know more about how cells sense their environment, but in general it appears we have made a real difference in the structural integrity of skin.”

Ninety-eight percent of women undergoing breast augmentation surgery say the results met or exceeded their expectations, according to a prospective outcome study published in the May issue of Plastic and Reconstructive Surgery®, the official medical journal of the American Society of Plastic Surgeons (ASPS).

Women also report improvements in self-esteem and quality of life after breast augmentation. The study adds high-quality information regarding expected outcomes after breast augmentation – including recovery time and psychological benefits.

98 Percent of Women Satisfied with Breast Augmentation Results

The survey study evaluated 225 consecutive women who returned for interviews at least one month after breast augmentation over a five-year period. Interviews included questions about the recovery, results, complications and psychological effects.

The average patient was 34 years old, and the majority of women received cohesive gel breast implants. The average implant volume was 390 cc.

Eighty-five percent of women rated their new breast size “just right.” Thirteen percent would have preferred a larger size and less than two percent a smaller size. Seventy-five percent of women rated their breast firmness “just right.” When asked to rate the result, over half of the patients gave it a perfect 10 on a scale of 1 to 10. Almost all patients (98 percent) reported that the results met or exceeded their expectations.

Increased Self-Esteem and Other Psychological Benefits

When asked about psychological effects, 92 percent of women reported improved self-esteem after breast augmentation and 64 percent reported an improved quality of life. Before surgery, the majority of women (86 percent) were self-conscious about their breasts.

Danger on the rise
There are individuals giving out all sorts of illegal cosmetic procedures, from Botox injections to dermal fillers. However, there is one procedure that seems to dominate the news: buttock injections. USA Today recently ran a report about the number of such cases and what may be behind the increase.

The most recent case involves a Florida man named Calvin Butler, who is being referred to as the “motel surgeon,” according to the Sun Sentinel, a Fort Lauderdale, Florida, newspaper.

Butler was sentenced to eight years in Prison on August 28 after he was found guilty of injecting two people, a man and a woman, with substances that he claimed would enhance their buttock, then sealing the injection sites with Krazy Glue. According to the news source, he charged each of those patients $200, and authorities expect others to come forward. One victim told police that he suffered an infection that required surgery following his procedure.

Use of the Acellular Dermal Matrix in Revisionary Aesthetic Breast Surgery

The successful use of Acellular Dermal Matrix (ADM) products has been reported in a wide range of clinical settings, including abdominal wall repair, hernia repair, facial and eyelid surgery, cleft palate repair, soft tissue augmentation, tendon repair, ulcer repair, vaginal sling repair, and breast reconstruction.

Immediate breast reconstruction using tissue expanders or implants has become one of the most commonly used surgical techniques, which has made visible rippling and contour deformity a more frequently encountered problem. Therefore, the applications of ADM to breast reconstruction have been of particular interest to plastic surgeons. The recent addition of allogenic tissue supplements avoids the problems of autologous tissue coverage and provides camouflage, thereby decreasing rippling and increasing soft tissue padding. This rising demand, coupled with good outcomes in breast reconstruction, has spurred tremendous interest about ADM use in aesthetic breast patients, particularly in the management of Capsular Contraction (CC). In the past, revisionary surgeries for Capsular Contraction  were generally performed with a total capsulectomy, removal of the implant from the subglandular plane, and the placement of a new implant in the subpectoral position.

This is a fairly simple procedure involving a change in implant placement from over the muscle to under the muscle. More recently, it has become necessary to perform revisionary surgery on volume-depleted breasts as a result of large implants that were placed under the muscle or severe scarring in the breasts. We remain plagued with a paucity of tissue, leading to the use of allogenic materials such as Acellular  Dermal Matrix (ADM). Our experience with patients who had their implant-related complications successfully corrected by a site change and the use of an ADM—provides a number of important conclusions.

First, the use of ADM enhanced soft tissue thickness or cushioning, decreasing and eliminating the visibility of implants.

Second, the use of these products increased our ability to successfully manage implant displacements and their recurrence.

Third, lower pole thickness was enhanced in patients with dual plane or neopectoral pocket conversions, especially in those undergoing concurrent mastopexies, interposing regenerative tissue between skin closure and implant.

The addition of the ADM to the management of breast augmentation intricacies gives us an exciting and, according to our data, reliable new surgical option. It is evident that by replacing the missing tissue at the implant interface, we are able to create an environment that is conducive to healing without excessive scarring Just as with any process that leads to excessive scarring, the key is to control the inflammatory phase and allow the healing tissue to transition quickly and smoothly to the next (proliferative) phase. It is also important to keep in mind that the true etiology of Capsular Contraction remains multifactorial and, by controlling all of the factors, we are able to see the future in prevention of this complicated and prolonged issue.

ADM is not a miracle product that can eliminate CC, but it serves as an adjunct to the already well-described principles in the management of CC, such as aseptic techniques with appropriate antibacterial pocket irrigations, postoperative drains, and the placement of textured implants. We postulate that in order to have a successful outcome with an ADM, the creation of a new pocket (raw surface) is essential to augment the interface.


ADM products show promise for applications in revisionary breast surgery and specifically for the treatment of complications of breast augmentation. This report describes different uses of the ADM that allow for the correction of established CC, malposition, and other implant related issues after previous subglandular or subpectoral breast augmentation.

Following pregnancy, despite regular diet and exercise, some women may find that they have excess skin in the lower abdomen, and others may find that they have breasts that have lost volume and “sag,” even after returning to their pre-baby weight. A mommy makeover describes the surgical procedures done to correct these changes and may include an abdominoplasty (also known as a “tummy tuck”), liposuction, and either a breast lift, breast reduction or breast augmentation.

When it is time to consider surgery, I urge all patients to do their homework before having surgery. Make sure that you see a qualified surgeon who is board certified by the American Society of Aesthetic Plastic Surgery. Ask where the surgery will be performed and make sure that it is an accredited facility. Ask to look at photos and make sure that you like and trust your surgeon.

To ensure that you have a long-lasting result, you should be at your goal weight for at least six months and have stopped breast-feeding for 6 months. It’s important to have stopped breast-feeding because your breasts need to return to their normal size and shape and stop producing milk. Additionally, I advise patients to have completed childbearing before proceeding with a “mommy makeover.” You can have a successful pregnancy after a “mommy makeover,” but you may require more procedures if you have more children or large weight fluctuations after your initial surgeries.
